£38.00 per hour
South East England
37 hours p/w, 2 days in the office and face-to-face visits

An exciting opportunity has arisen for a driven and skilled Advanced Practitioner to join a dynamic Continuing Healthcare (CHC) Team in the South East! If you're passionate about making a difference and thrive in challenging, rewarding roles, this could be the perfect fit for you.
 
What’s in It for You? This full-time permanent position puts you in the heart of pivotal decision-making processes within the CHC framework. You’ll play a vital role in shaping outcomes for the Local Authority, attending assessments, representing the LA in disputes, and contributing to Decision Making Panels with your expertise.
 
What Will You Be Doing?
-             Take the lead in CHC assessments (DST meetings), confidently representing the Local Authority's position.
-             Prepare impactful position statements and advocate for the LA in disputes.
-             Objectively review evidence and make informed, evidence-based decisions, all while supporting the Local Authority’s perspective.
-             Support the wider ASC staff cohort with day-to-day CHC challenges, questions and deliver CHC training.
 
About You
You’re not just well-versed in the Continuing Healthcare Framework—you’re confident in applying it to real-world challenges. Your keen eye for detail and commitment to evidence-based decision-making will set you apart. In this role, your voice matters, and your insights will drive positive change.

What you need:
  • A degree in Social Work or equivalent
  • Registered with Social Work England
  • Highly experienced within CHC
  • Completed relevant post-qualifying training
  • Enhanced DBS check
  • Excellent communication and written skills
