Personal Advisor

Pertemps are recruiting for experienced Personal Advisors in North London within a leaving care service.

This role is to start ASAP and you will be working in partnership with the young people as they transition from care and into independence or adult services. Your responsibilities will include:

• Working with young people who are Looked After and Care Leavers, aged 16 to 25 years.
• Working with unaccompanied Asylum Seekers.
• Providing a holistic service to young people, assessing needs to provide the best possible support, advice, guidance and planning.
• To work in partnership with key agencies and support networks.
• To complete Pathway Plans that are robust, reflective and based upon individual needs.
• To support young people in making wise choices, being safe and reaching their full potential.
• Supporting the housing needs of our young people making their transition into adulthood.
• Supporting our young people in meeting their educational potential and engagement in education, employment and training.
• Supporting our young people to meet their health needs.
• To ensure or young people wishes, and feeling are at the centre of all practice and that their views help to shape interventions and practice. Which is recorded within their Pathway Plan.
• To ensure that accurate records are maintained which reflets decision making.
• Support the team and young people through the duty service and social groups.
