Experienced Social Worker required to join our client's Adult Community Team on a temporary basis

About the Social Worker:

• Working in the community with adults aged 18yrs and over. To be able to undertake Care Act Assessments, Mental Capacity assessments & Best Interest outcomes
• Complete DOLs assessments, experienced with CHC checklists and DST's
• Experienced social worker with a good track record to be able to operate from a Strengths Based perspective, demonstrating Least Restrictive Practice
• Support the team duty function by triaging referrals into the team
• Effectively prioritising their day to day case management activities
• Confident in working with professional partners in an MDT approach
