Job Description

Qualified Social Worker - Older People & Physical Impairment
£38.15 per hour
Cardiff, Wales

About the role:

To undertake scheduled reviews and monitoring visits to residential and nursing homes.
To promote and enhance the lives of people living with dementia within a residential/nursing placement.
To work with providers to improve the opportunities for people living with dementia within a placement setting.
To provide reports to the Team Manager on visits to placement settings.
To liaise with other professionals and out health colleagues.

What you need:

A degree in Social Work or equivalent
Registered with Social Care Wales
at least 3 years post qualifying experience as a social worker & not left a LA permanent contract in Wales within the last 6 monts
Completed relevant post-qualifying training
Enhanced DBS check
Excellent communication and written skills
