Vitalis are recruiting for an experienced Social Worker to work in a Mental Health Team in Liverpool.
To contribute to the operational delivery of an effective Adult Social Care Service as part of an integrated Community Mental Health Team responsible for safeguarding, support planning and promoting the wellbeing of vulnerable adults /older people continuously improving outcomes. To identify individual needs through assessment. To plan and co-design individual support plans. To ensure that regular monitoring and reviews of care plans are carried out. To be responsible for social work provision including managing own caseload whilst managing the reputation of the Department and Council and complying with national and service procedures and guidance. We are looking for a social worker that is experienced in application of the Mental Health Act, Mental Capacity Act and Care Act. You should have experience of hospital discharge from a Mental health ward, experience of attending tribunals and completing social circumstance reports as well as assessing and sourcing support based on individual need.
