Vitalis Social Care are currently recruiting for an experienced Social Worker in Rochdale to join an Integrated Neighbourhood Team.
Our integrated neighbourhood teams are at the heart of our adult care service, delivering and reviewing innovative services for our residents, helping to reduce health inequalities, enhance life and support independent living. We work across friendly, dynamic teams alongside our district nurse colleagues with a focus on partnership working to achieve best practice and desired outcomes for our residents.
