Qualified Social Worker, Hospital Discharge Neighbourhood Team
Pay rate £35 per hour
The Talent Crowd are recruiting for an experienced Social Worker to work within a Hospital Discharge Neighbourhood Team in Leeds.
This role is for three months only ideally the start date would be the 23/12/24 or 30/12/24.
The role of the senior social worker would be as follows:

• To take a key role in supporting the team alongside the team manager including supporting less experienced staff with complex cases.
• To support with allocations and attending partner meetings in the absence of the team manager.
• To be responsible for own case load which will be based both in hospital and outside of hospital. Such work will include supporting safe and timely discharges from hospital and reviewing care in temporary placements and in people's homes.
