Pay rate - £28 per hour
Vitalis are looking to recruit Adult Social Workers to work in a Transitions Team in Lewisham. 
Experienced social worker for the AWLD Transition service which is an 18-25 team based in adult social care. Social needs experience of working with vulnerable adults, must have experience of working with YP with a learning disability, autism, physical disability. 2 years post experience Strong strength-based assessment skills, understanding of DoLS and MCA and ability to complete Com DoLS, must be legally literate. Experience of working in a similar environment essential.
