Working Pattern: Hours: 36 hours per week  
 On-site at St Mary’s Hospital £32.55 per hour   
We are seeking an experienced and dedicated Hospital Social Worker to join our St Mary’s Hospital Social Work Team, working closely within the A&E department. This role is pivotal in supporting timely and safe hospital discharges and ensuring residents receive the care and support they need. Key Responsibilities: Triaging referrals on behalf of the Tri-Borough Hospital Discharge Team Completing quick assessments for residents of the Tri-Borough Undertaking Care Act Assessments and developing Support Plans Completing Safeguarding Concern Assessments (Section 42.1) Setting up care packages to facilitate safe discharge Providing information and signposting to A&E staff
