Adult Social Worker, Shropshire 
Umbrella £28.40 per hour capped rate in line with the MOU.  
We are seeking a motivated Social Worker to conduct assessments to determine individual's short and long-term needs from hospital. Responsibilities include but are not limited to: conducting and participating in MDTs, Care Act Assessments, Mental Capacity Act Assessments, ICB interface for CHC funding (including interim health funding, CHC checklist and Decision Support Tool meetings); care planning and completion of support plans; participating in duty tasks within the community team on a rotational basis; and rotational weekend working
