We are seeking qualified social workers to join our team in the Hospital Discharge team at Princess Royal Hospital and Royal Shrewsbury Hospital. The primary goal is to provide the best service for individuals who require support with complex discharge and admission avoidance from the Emergency Department and Frailty Assessment Unit under the principles of the Care Act (2014). The model we support aims to get people home as soon as possible to improve length of stay, enhance flow in the hospital and as a system, support the hospital to have the best performance and quality for the people who need acute intervention.  
 
Royal Shrewsbury Hospital Guildhall, Shrewsbury Princess Royal Hospital 
 
 Pay - £up to 38ph
