Adult Social Worker, Hospital Assessment Team, Neath Port Talbot 
Pay rate £38.76 per hour  

The role  for an agecny worker has become available to join a committed and motivated Hospital Assessment Team who work hard to deliver excellent standards in a positive working environment. The Hospital Team works in partnership with individuals/ multi professionals to identify what matters to them. This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital.
