Pay rate £40 per hour, 30 hours per week

This is work as the Team Leader for a Hospital Discharge and Post-Discharge Assessment Team 
There is an expectation to be on-site for 3 days across Calderdale Royal Hospital, Huddersfield Royal Infirmary, and Princess Buildings in Halifax.
